GoldWorks® Disconnect Instructions – Lease Line and Dial-Up Service

To disconnect your Leased Line Service:

1. Complete the GoldWorks® Leased Line Disconnect Request form [DOC 37K].
   • Prior to submitting the disconnect form, it is advisable to contact other departments within your company to verify that the leased-line is not utilized.  This will prevent possible service disruptions and help to avoid service re-connection fees.
   • Complete all fields on the form.
2. Submit completed form via fax to (703) 918-8003, or mail original form with signatures to:

Please note that the Lease Line Disconnect Request may take up to 60 days to process.

To cancel your Dial-Up Service:

1. Complete the GoldWorks® Dial-Up Cancellation Form [DOC 33K] or submit your intent to cancel your GoldWorks dial-up line in a letter using your company letterhead.  Please provide your GoldWorks account number, company address, contact name, and phone number in case we need to reach you.
2. Submit form/letter via fax to (703) 918-8003, or mail original form/letter to:
